Personnel Action Forms
Personnel Action Forms are to be used by managers when requesting additional staff, for staff replacements or salary changes. The Personnel Action Form will be completed electronically, and may be saved during the process for completion later.
Personnel Action Form - EXPRESS
The EXPRESS version of the Personnel Action Form may be used by managers when requesting Title Changes, FOAP changes, or Recurring Stipends.
