The following form is to be used for establishing goals, preparing employee evaluations, and rewarding outstanding performance
Annual Performance Evaluation Form
The form should be used for salaried, non-union employees. The review period covers April 1, 2022, through March 31, 2023. The performance discussions with employees should be conducted beginning April 1st through April 30th. The process is initiated through the link below via Docusign. Supervisors will complete a form for each employee to be reviewed.
2023 Performance Evaluation Form – For this form, supervisors will complete the employee to be reviewed, and include their own name and email, and their division head or managers. Docusign will then route to the employee first to complete, then it will be returned to the supervisor to review and complete. Docusign will automatically route the form to the next person to complete and/or review the performance evaluation form.
