The following form is to be used for establishing goals and preparing employee evaluations.
Annual Performance Evaluation Form
The form should be used for salaried, non-union employees. The review period covers April 1, 2023, through March 31, 2024. The performance discussions with employees should be conducted beginning April 1st through April 30th.
The 2023-24 Performance Evaluation Form may be completed as a fillable pdf (requires a separate copy for employee and supervisor) or a fillable word document. The completed and signed review, including a job description, must be scanned and emailed to HR at hr@lafayette.edu.
2023-24 Performance Evaluation Form– fillable pdf If you use the fillable pdf, the employee and supervisor each complete their sections on separate forms. (employee – section 1; supervisor sections 2-6) Save each as a pdf and print both copies. The Summary Rating and Approval signatures must be completed via paper copy.
2023-24 Performance Evaluation Form – fillable word document If you use the fillable word document, only one copy is required. The employee can complete section 1, save and send to the supervisor, who completes sections 2-6). The Summary Rating and Approval signatures must be completed via paper copy.
